The following forms are downloadable as .pdf files. These forms are required for STM trips with SIFAT.

They may either be printed, filled out by hand or typed and mailed, or filled out on your computer and sent as e-mail attachments by following these printable instructions.

Both team members' forms and team forms must be submitted to Peggy Walker at least one month before your scheduled departure. These forms are needed for your travel insurance and in-country preparations.

You must take a copy of these forms and give them to the in-country SIFAT representative upon arrival in Bolivia or Ecuador. Team leaders will also want to keep a copy on file while traveling (especially the parental consent form, as airlines have the right to ask for it).
